You’ll also need to sign a medical release form providing healthcare professionals to share your medical information with relevant parties. Additionally, if you’re participating in the Child Dental Benefit Schedule, we’ll also require a signed consent form as acknowledgement of bulk billed treatment.

Child Dental Benefit Schedule Bulk Billing Consent Form
We require a signed consent form as acknowledgement of bulk billed treatment and any fees it may incur.
A medical release form is a document that gives healthcare professionals permission to share patient medical information with other parties.
We respect that your time is valuable and we appreciate that you understand ours is too.
We have in place a 24 hour cancellation policy. This means that any cancellations that occur 24 hours or more before the scheduled time are able to be re-booked at no additional cost to the patient. The first time a cancellation is made within the 24 hour period prior to the time of the booked appointment, the patient will be given the opportunity to re-book (with no financial penalty applied). Any subsequent cancellations by or for the same patient that occur within the 24 hour period may result in a cancelled booking fee being applied (which will be equal to the value of the consultation fee for that booking which had been made and subsequently cancelled).
Patients who make a booking and subsequently fail to present for their designated appointment (and have not contacted the practice to cancel their appointment), may receive a cancelled booking fee.
